The reading form asks for first name, selected article topic and a separate policy acknowledgement. Last name, reading stage, email, mobile number and ZIP are optional. Phone is needed only if you select SMS. Do not submit identity documents, account credentials, medical records or financial statements. The plain contact form has no sending endpoint; use the listed support contact details instead.

Topic choices identify reading requested through Question labels. They are not used to make clinical, financial, eligibility or coverage decisions. Health-related selections are not shared for targeting. No third-party lead matching is offered.
